Benefits and Risks
Telehealth improves access to care but has limitations, including the absence of an in-person physical exam and possible technology failures. A provider may determine that in-person care is needed.
Your Responsibilities
You agree to provide complete, truthful health information, follow your provider's instructions, and seek in-person or emergency care when necessary.
Right to Withdraw
You may withdraw consent and stop telehealth services at any time without affecting your right to future care.
